Humana’s home-based care programs aim to reduce costs and improve quality of care by delivering convenient services in the comfort of members’ homes or virtually.

As part of this journey, members can expect:

  1. An In-Home Health and Well-being Assessment (IHWA):

    Members receive an in-home visit by a licensed physician and/or clinician who conducts an assessment to:

    • Identify gaps in care
    • Provide clinical diagnoses
    • Deliver prescription assistance
  2. Care coordination:

    The care team coordinates across resources, delivering seamless care.

  3. Whole-person support:

    Humana’s home healthcare program utilizes care resources covering a range of areas, including:

    • Primary care
    • Behavioral health
    • Complex care

How all stakeholders benefit

As the senior population grows, home healthcare is becoming an increasingly important frontier. A focus on high-quality, whole-person care in a non-clinical setting allows seniors to enjoy a better quality of life while also benefiting plan sponsors and the healthcare system in several ways, including:

  • Better patient experience:

    Solutions provide members with greater convenience while reducing barriers to care. Member satisfaction is reflected in the high customer satisfaction scores members give to Humana’s home health solutions.

  • Improved engagement:

    More than 30% of Humana’s MA members have two (or more) social determinants of health needs. Home healthcare offers a deeper understanding of members’ circumstances and strengthens the provider/patient relationship.

  • Improved outcomes:

    Members who are cared for by high-quality home health providers have about 11% fewer hospital readmissions, underscoring the emphasis on preventive, proactive care.

Achieving results

Aging in place through home-based care delivers the following results:

  • Cost savings:

    Costs are lower when compared with the cost of care in institutional settings. This is proven by a study of Medicare beneficiaries discharged to post-acute care. The study found that costs were significantly lower for beneficiaries who were discharged to home healthcare compared with those who were discharged to a skilled nursing facility.1

  • Reduced care gaps:

    Home healthcare has played a role in closing STARS gaps in the areas of diabetes, hypertension and medication adherence. Specifically, CenterWell Home Health® is successfully piloting to help Humana close care gaps by meeting certain healthcare needs within a home setting.

  • Stronger data:

    Patient data is more robust for Humana members who use CenterWell Home Health—this allows Humana to better meet members’ care needs with personalized care plans.
